Trump Announces $90 Medicare Payments for 20M Seniors
President Donald Trump announced that the federal government will send one-time $90 payments to more than 20 million seniors enrolled in Medicare to help offset the cost of their monthly Medicare Part B premiums. The payments will come from the $2 billion Medicare Improvement Fund and will go out in early October via direct deposit or mailed check. Most Medicare Part B enrollees qualify, though those with Medicaid premium assistance, higher-income enrollees paying income-adjusted premiums, and Medicare Advantage enrollees do not.
What did Trump announce for Medicare seniors?
Late Friday, Trump said on social media that the government would send the $90 one-time payments to more than 20 million Americans enrolled in Medicare, the federal health insurance program for people aged 65 and older and some younger people with disabilities. The goal, according to the White House, is to help seniors offset the cost of their monthly Medicare Part B premiums.
The White House released details on its website saying most eligible seniors will receive the payment as a direct deposit of $90 in early October. Seniors without direct deposit on file will receive a check, also sent in early October, at the mailing address they have registered with Medicare. A Centers for Medicare and Medicaid Services fact sheet put the target delivery date at on or around Oct. 8, 2026.
Who qualifies for the $90 Medicare payment?
According to the White House and the Centers for Medicare and Medicaid Services:
- Most Medicare Part B enrollees are eligible — Part B covers doctor visits, outpatient hospital services, medical equipment, and physician-administered drugs.
- Seniors must be living in the United States.
- Seniors whose Part B premiums are already paid by Medicaid do not qualify.
- Higher-income enrollees who pay an Income-Related Monthly Adjustment Amount (IRMAA) — income-adjusted premiums — are excluded.
- Seniors in Medicare Advantage plans, the private-plan alternative to traditional Medicare, are not eligible. According to the nonpartisan health research nonprofit KFF, more than half of eligible Medicare beneficiaries have Medicare Advantage.
The announcement covers more than 20 million people — not all of the roughly 64 million elderly Medicare enrollees, as CNN reported. The payment is automatic: eligible seniors do not need to submit a separate application. Seniors can call 1-800-MEDICARE (1-800-633-4227) to check eligibility and the Social Security Administration at 1-800-772-1213 for payment status, per the White House fact sheet.

Where is the money coming from?
The payments will come from the Medicare Improvement Fund, which Congress established in 2008 as flexible funding to improve program operations and payments to health care providers. The White House says the fund holds about $2 billion and that no administration has ever utilized it — making Trump the first president to draw from it directly for seniors’ costs.
Health policy experts note the move is unusual. Larry Levitt, KFF’s executive vice president for health policy, told the AP that previous administrations have not used the Medicare Improvement Fund this way, although Congress has periodically tapped it for other priorities — for example, $20.74 billion was used in the creation of the Affordable Care Act in 2010. “The Medicare Improvement Fund language is quite broad and gives substantial discretion to the administration,” Levitt said.
Why is Trump sending these payments now?
The announcement is the latest in a series of financial promises Trump has made to voters ahead of the Nov. 3 midterm elections, where control of Congress is at stake. Trump’s Republican Party holds narrow majorities in both the Senate and the House. Polls cited by the AP show Americans increasingly blame Trump for the high cost of gasoline and groceries, with just 17% approving of his handling of inflation.
Other recent payment promises include:
- $500 ACA rebate checks for nearly 1 million people who bought health insurance through the Affordable Care Act, described by the White House as refunds for excessive fees. Trump said at a Saturday rally in Vandalia, Ohio, that 70,000 families in Ohio would receive those rebates “within the next couple of days.”
- $5,000 checks to all adult citizens if Republicans retain control of the House and Senate — a pledge analysts say would likely cost more than $1 trillion and would require congressional approval.
- $1,776 “warrior dividend” payments to active-duty military members, announced in December.
At the Ohio rally on Saturday night, Trump highlighted the $5,000 promise and told supporters that the ACA rebates would arrive “within the next couple of days,” while musing about the military payments’ timing: “Maybe I should have waited a year… Should I wait for before the election?”
The announcement also lands as Medicare Part B premiums have climbed. CNN has reported that Part B premiums jumped nearly 10% ($17.90) from 2025 to 2026 — the largest increase in four years — with the standard monthly premium now at $202.90.
